What to Prepare
New Patient Forms (Click here to log into your patient profile and fill out forms prior to your appointment)
Insurance card information (company name, subscriber ID & group number ready before your appointment will help things go smoothly)
Driver’s License
Please bring records & x-rays (within 1 year) from previous dentists if available
What to Expect
For all our new patients, we start with a comprehensive exam with x-rays to determine your dental needs. We will work with you to develop a treatment plan that is just right for you. We aim to provide comprehensive treatment to help you obtain oral health and achieve the smile you’ve always dreamed of.
To help make your appointment flow more smoothly, we ask that you fill out the New Patient Forms prior to your appointment, or please arrive about 15 minutes before your appointment to allow for time to fill out the paperwork in-office. You will receive appointment reminders leading up to your appointment, starting with 1 week email reminder and subsequent text message reminders.
Insurance Plans Accepted (PPO or Preferred Provider Organization)
We accept most PPO dental plans. Please contact us to make sure we are in-network with you prior to your appointment.
No Insurance? No problem!
There is no need to cut corners on your oral health because you don’t have dental insurance. you can save money on all your dental treatments with our In Office Benefit Plan*! This plan was developed for patients who do not have dental insurance or have a high monthly premium. Your benefit plan becomes active the same day you make your annual payment and is active for 365 days from date of sign-up. There are NO waiting periods, NO deductible, and NO yearly maximums.
Option 1: Prophylaxis Plan (for patients with healthy gums) - 2 Prophylaxis dental cleanings per year
Option 2: Periodontal Plan (for patients with gum disease) - 3 Periodontal Maintenance dental cleanings per year
Both plans include:
2 Complete annual dental exams
All necessary x-rays needed to complete annual exams
2 fluoride varnish applications
1 Complimentary Emergency exam
15% savings on general dental procedures (e.g. fillings, deep cleanings, nightguards)
15% savings on cosmetic services, including dental veneers and custom whitening
15% savings on major dental procedures (e.g. crowns, bridges, implants & implant restorations, extractions & periodontal surgeries)
